Do you remember 2020?

while their family members looked on,
watching through their phone screens.
Healthcare workers quitting.
Called it unprecedented.
It was a once in a generation
catastrophe, and in many ways, COVID was.
But that's not the reality In 1347,
there was something that would
make COVID look like child's play.
1347 is when it began in earnest,
at least per our historical records.
A Genoese trading ship pulls into the
port of Messina, Sicily, south of Italy.
Sailors on board are dying,
covered in black, pus-filled
swellings the size of eggs.
Harbor masters order the ships out of port
immediately, but it's already too late.
What follows will kill between thirty
and sixty percent of Europe's entire
population, some regions losing half
their people in a matter of months.
Cities are left with
more dead than living.
Fields go unplowed.
Churches run out of burial grounds.
We're talking about truly the deadliest
pandemic in recorded human history.
We are talking about the Black Death.
So why should we care about a
fourteenth-century apocalypse when
we're still processing our own?
It's because when you look closely at how
medieval Europe responded to the plague,
the panic, the denial, the scapegoating,
the acts of extraordinary courage, you
start to feel something uncomfortable.
It's recognizable.
Science obviously has advanced since 1347.
We have germ theory, vaccines,
antibiotics, epidemiology.
We understand at a microbial level what
the Black Death was and how it spread.
The people of medieval Europe had none
of that, and yet their responses and
ours were in so many ways identical.
Because while science progresses, human
nature appears to be a bit more stubborn.
We will delve into not just the science
and history of a plague, but we will look
in a mirror, one that reflects something
true and yet unsettling about who we are.
When we talk about history at this
scale, to retreat into the numbers,
percentages, death tolls, statistics
that are frankly too immense for
the human mind to truly absorb.

no science, no reassurance that this
would end, just wave after wave of death,
arriving without warning, taking the
old and the young and the strong alike,
offering no logic and certainly no mercy.
We tend to think of people from the
distant past as somehow other, as
figures in illuminated manuscripts,
stoic and two-dimensional,
but of course they were not.
They loved and feared and
grieved exactly as we do.
Their suffering was as real,
as raw, and as desperate as
anything that we have ever known.
These people are us, separated
by seven centuries, but not
by a single human feeling.
So as we explore the history, the
science, the politics, and the strange,
sometimes shocking ways that medieval
Europe responded to the catastrophe,
let us never lose sight of that.
Let us carry their suffering with us and
treat it with the dignity it deserves.
Because that, perhaps, is the
most important reason of all
to care about the Black Death.
Let's get into our time machine.
It's the summer of thirteen forty-eight
in a village in southwest England.
Elizabeth, a young
peasant woman, is frantic.
She's heard rumors of a pestilence
marching inland from the coast.
But just maybe with enough
prayer, divine judgment will
pass over her and her family
Not the fatigue of a long day's work.
This was different This was a tiredness
that was deep into her bones, a heaviness
behind the eyes, a chill that created
rigors never to feel warmth again.
The next day, however, the
fever would arrive in earnest,
savage and climbing fast.
Her skin would have burned.
Her head would have pounded, sensitive
to the light, wincing, painful eyes.
Every joint in her body would
ache she was being wrung out.
And then she noticed the swelling beneath
her arm or perhaps in her groin or at
the side of her neck, a lump hot and
hard rising quickly beneath the skin.
And the pain would be extraordinary,
a sharp, intense pressure that made
it unbearable to move limbs, whether
her arm or her leg, or struggling
to find any position of relief.
This was the bubo, the
defining mark of the plague.
Named from the Greek for groin, filled
with bacteria, with dead tissue, with the
debris of her own immune system fighting
and losing a war it could not win.
The bubo, if she was fortunate,
in the most terrible sense of the
word, might have burst, rupturing
through the skin in a flood of black
and bloody pus, Some survived this.
The release of pressure occasionally
allowed the body a fighting chance.
But likely, she was not so fortunate.
By the fourth or fifth day, the
bacteria would have breached the
walls of her lymph nodes entirely
and entered her bloodstream.
This is septicemic plague, and
it transformed her suffering into
something altogether more catastrophic.
Beneath her skin, her blood vessels
would begin to rupture and hemorrhage.
Dark blotches bloomed over her
body, the black patches that
gave the Black Death its name.
Her fingers and toes, starved of blood,
would begin to blacken at their tips.
Gangrene.
By now, she would have been
profoundly delirious, her heart
racing but faint at the same time.
And then she coughed.
The bacteria had reached her lungs.
What had begun as bubonic plague was
now pneumonic plague, the most lethal
form of all and the most contagious.
With each cough, she expelled a
fine mist of infected droplets and
saliva into the air around her.
Anyone who breathed that air,
had now inhaled Yersinia pestis
directly into their own lungs.
Her cough would become bloody,
pink at first and then darker.
Her breathing would become labored,
a wet, rattling struggle as
her lungs filled with fluid and
tissue destroyed by the infection.
She would have been suffocating slowly,
conscious enough in her clearer moments
to understand what was happening, too
weak to do anything but endure it.
She would not have had a name
in the historical record.
She was not a queen or a noblewoman
whose death was noted in a chronicle.
She was one of the millions, a wife,
a mother, a daughter, a neighbor,
whose suffering left no trace except
in the silence she left behind.
To understand how the Black Death
swept across the known world with such
devastating speed, we first need to
understand the organism behind it.
And to do that, we need to
go to somewhere unexpected.
We need to go to the rats.
The bacterium responsible for plague
is called Yersinia pestis, named
with a certain grim poetry after the
Swiss-French bacteriologist Alexandre
Yersin, who first isolated it in
1894, more than five centuries after
the Black Death had done its worst.
It seems by any measure it has been one
of the most effective killing machines
in the history of life on Earth.
But what makes it truly remarkable, truly
dangerous, is not what it does when it
is rampaging through human populations.
It is what it does the rest of
the time, which is mainly nothing.
Yersinia pestis lives quietly, persists in
what biologists call a natural reservoir,
stable population of wild rodents and
the fleas that feed on them, in which
the bacterium circulates at low levels
without causing catastrophic harm.
The rodents carry it, the fleas transmit
it between rodents, and the system
maintains a kind of equilibrium, a
biological detente, in which host
and pathogen have reached over
thousands of years of co-evolution
a state of uneasy coexistence.
This is known as an enzootic
plague, enzootic from the Greek
meaning disease within animals.
causing no threat to human beings.
The infected fleas have no
particular reason to seek a new host.
Their rodent populations are stable,
relatively resistant, and very much alive.
A flea, after all, simply
wants its next blood meal.
And if that meal is readily available
on a living, warm-blooded rodent a
few inches away, it will take it.
But nature is not always
stable, and this is where the
story becomes deeply sinister.
When Yersinia pestis, through migration,
climate pressure, human encroachment,
or simple ecological change, finds its
way into a rodent population that has
not had the same centuries of exposure,
the consequences are catastrophic.
These naive populations, species such
as prairie dogs, rock squirrels, and
critically the black rats that lived
in intimate proximity to medieval
human settlements, have no developed
resistance, no evolutionary memory
of this pathogen, and so when the
bacteria arrives among them, they
die rapidly and in large numbers.
This changes everything, because
now the fleas, the essential
vector, the biological bridge
between the bacterium and its host,
find themselves in a conundrum.
Their hosts are dying beneath them.
The warm body they depended upon
is cooling, and a cooling body
is a flea's red flag, a signal to
leave to find another source of
warmth, another source of blood.
Desperate and hungry and carrying a lethal
payload of Yersinia pestis in their gut,
the fleas begin to seek alternate hosts.
Other rodents, yes, accelerating the
die-off further, but also larger mammals,
livestock, pets, and human beings.
The transition from a stable contained
infection circulating quietly in a
resistant animal population to an
explosive outbreak jumping to new
species is known as epizootic plague.
And it is at this threshold, at this
precise biological moment, that plague
stops being a problem for rodents
and starts being a problem for us.
And as it feeds, it regurgitates
because Yersinia pestis actually
blocks the flea's digestive tract.
It's a flea bowel obstruction, causing
it to feed more desperately to expel
bacteria-laden material back into
the bite wound with each attempt.
The bacterium enters the bloodstream,
and from that single, almost invisible
moment, a flea bite barely felt,
easily ignored, the cascade commences.
To understand why the Black Death
killed with such efficiency when it
finally arrived in Europe, we need
to step back from the plague itself
and look at what Europe had already
been through in the decades before
the first ship docked at Messina.
Because by 1347, Europe was
not a continent in good health.
It was a continent already in duress.
The story begins not with
bacteria, not with rats and fleas,
but actually with a volcano.
In the closing years of the 13th
century, the world's climate changed.
The trigger was geological,
catastrophic, sudden, and halfway
around the world from the fields of
England, Italy, and France, where its
consequences would be felt most acutely.
On the Indonesian island of Lombok,
a volcano now known as Mount Samalas
erupted with a violence that dwarfs
almost anything in recorded human history.
Geologists and ice core analysts
studying the evidence centuries
later would conclude that it was
the most powerful volcanic explosion
since the end of the last ice age.
Enormous volumes of ash and sulfur dioxide
were blasted into the upper atmosphere,
forming a veil around the Earth that
filtered and scattered the sun's energy.
Temperatures fell, not dramatically,
not the sudden freeze of a
science fiction movie, but enough.
Enough to shorten summers
across the Northern Hemisphere
by critical days and weeks.
Enough to deepen and extend
winters that were already harsh.
Enough to alter the delicate
seasonal rhythms upon which all
medieval agriculture depended.
In Europe, the consequences
were immediate and devastating.
The summers that followed were cold and
wet, relentlessly, unseasonably wet.
Rain fell through what should have been
the growing season, waterlogging fields,
rotting crops in the ground before
they could be harvested, turning the
rich agricultural lowlands of England,
France, the low countries in northern
Germany to a sodden, unproductive ruin.
Year after year, the harvest failed,
not partially, not in isolated
regions, but across the continent,
simultaneously on a scale that had no
precedent in living memory at that time.
This period is known to historians
as the Great Famine, and to call it
merely a famine is an understatement.
By the early 14th century, the people
of northern Europe were starving.
. Food prices spiraled beyond
the reach of the poor.
Grain stores already depleted by
successive failed harvests ran dry.
Animals that might have served
as a last reserve of protein, the
horse, the cow, were slaughtered and
eaten, destroying in the process the
very means of future cultivation.
And then truly desperate measures began.
Contemporary chronicles written by
monks and clerics record what happened
in the communities around them
with a kind of horrified restraint.
People ate cats, they ate dogs, they
stripped bark from trees and boiled it.
They consumed bird droppings,
rotten grain, whatever could
be forced into a stomach for
sustenance for one more day of life.
In the most extreme cases, and the
chronicles , are careful to note
that these are aberrations, though
they do appear with some troubling
frequency, that people ate the dead.
Parents, it was recorded, concealed the
deaths of children to preserve the food
that would otherwise have gone to them.
The living envied the dead
their release from hunger.
By 1317, this famine alone had
killed an estimated ten percent of
the population of Northern Europe.
One in ten people gone, not
from disease, not from war, but
from the simple absence of food.
And then just as the immediate crisis
of the Great Famine began to ease,
something more insidious took hold.
A chronic, persistent, low-level emergency
that would define life across much
of Europe for the next three decades.
Recurring harvest failures reduced
agricultural capacity from years of
neglect and the loss of working animals.
Populations weakened by years
of malnutrition meant that
full recovery never truly came.
The human cost of this prolonged
deprivation is difficult to
overstate in physiological terms.
Chronic malnutrition
suppresses immune function.
It stunts development in children,
leaving them permanently compromised.
A population that has spent 30
years hungry is a population
whose immune systems are depleted.
By the 1340s, the people of Europe
were exhausted, malnourished, crowded
into cities with no understanding
of sanitation, no clean water, no
medical science in any meaningful
sense, and no reason to believe
that the worst was behind them.
They were, in the most precise
biological sense, profoundly vulnerable.
The pathogen was already moving
westward through Central Asia.
It had no awareness of what it was
approaching, no cunning, no intent.
It was simply doing what pathogens do,
finding hosts, replicating, spreading.
Europe was ready, not for
survival, but for catastrophe.
We tend to think of catastrophe as
something that arise from the outside,
something alien, something other, a force
of nature indifferent and unstoppable.
But the Black Death did not simply
fall upon the world from above.
In a very real sense, human beings
carried it there themselves.
The very things that represented
the pinnacle of medieval progress,
exploration, commerce, empire,
connectivity, became the biological
highway along which Yersinia
pestis traveled from the remote
steppes of Central Asia to the
crowded ports of the Mediterranean.
For thousands of years before the Black
Death, Yersinia pestis existed in a
state of notable geographical isolation.
Its home was the vast, windswept grassland
of the Mongolian steppe and one of
the most remote and sparsely inhabited
landscapes on the face of the earth.
And its primary host was an animal most
people in the Western world had never
heard of and would never encounter.
The tarabagán.
Known in the West as the Mongolian
marmot, the tarabagán is a large
burrowing ground squirrel, similar
to the prairie dog in North America.
It's sociable, numerous, and
deeply embedded in the ecology
of the Central Asian grasslands.
It had coexisted with Yersinia pestis
for millennia in that same uneasy
equilibrium I described earlier.
The bacteria circulated, the marmots
carried it, the fleas transmitted
it between them, and the rest of the
world remained entirely untouched
because the tarabagán's world and
the human world barely overlapped.
We know this isolation was
already beginning to crumble
by the late thirteenth century.
Marco Polo, traveling the Silk Road on
his extraordinary journey to the court
of Kublai Khan, passed through the
Gobi Desert and noted in his account
the enormous populations of what he
called Pharaoh's rats, almost certainly
tarabagáns, swarming across the landscape.
. But the observation tells
us something important.
By the time of Marco Polo's journey,
the boundaries between the human
world and the world of the infected
marmot were already permeable.
Trade routes and travelers were already
threading through landscapes that
had previously been the exclusive
domain of the steppe and its wildlife.
And then came the Mongols.
The unification of the Mongol tribes
under Genghis Khan in the early
thirteenth century and the subsequent
expansion of the Mongol Empire under
his successors produced the largest
contiguous land empire in human history.
At its peak, it stretched from the
Pacific coast of China to the edges of
Eastern Europe, an unbroken corridor of
controlled territory that connected, for
the first time, the East to the West.
This was, in commercial terms,
an extraordinary achievement.
The Mongol Khans actively encouraged
trade along the Silk Road,
establishing the infrastructure of
long-distance commerce on a scale
the ancient world had never seen.
Rest stops, which had supplies
and shelter and fresh animals, were
built at regular intervals across
thousands of miles of territory.
And perhaps most remarkably, the Mongols
created an institution called the yam.
It was an early postal relay system in
which mounted riders carried messages and
goods at speed across the empire, changing
horses at dedicated stations, covering
distances that would have taken ordinary
travelers weeks in a matter of days.
It was breathtaking in its ambition.
It was the 13th century
version of the internet.
And like all networks, it was only as
safe as whatever was traveling along it.
The first human victims of Yersinia pestis
in this new era were almost certainly
Mongol herdsmen, nomadic pastoralists
who moved their animals across the
northern steppe in search of grazing land.
For herdsmen sleeping on the ground,
setting traps, skinning animals for their
valuable pelts, exposure to the infected
fleas was not a remote possibility,
it was an occupational hazard.
But the Tarbagan, for all its
biological culpability as the original
reservoir of Yersinia pestis, was
not the animal that carried the
plague to the rest of the world.
It was too specialized, too geographically
constrained, too tethered to its
particular corner of the steppe.
The Tarbagan would start the
fire, but it would not spread it.
For that, history required
a different animal entirely.
Enter rattus, commonly
known as the black rat.
If the terebagon was an obscure
supporting character in this story,
the black rat is its villain.
Ubiquitous, adaptable, and intimately
entangled with human civilization
in a way that no amount of effort
has ever fully managed to sever.
The black rat is, in the most admirable
yet appalling sense of the phrase, a
masterpiece of evolutionary engineering.
Unfortunately, it can climb, scaling
vertical walls, running along ropes
and rigging, ascending the inside
of a ship's mooring line with an
ease that seems to defy physics.
It can jump distances that bear no
reasonable relation to its body size.
To squeeze through gaps barely larger
than a quarter, it can gnaw through
wood, through lead, through the walls
of granaries and the hulls of ships,
through virtually any material that
stands between it and a food source.
It thrives in cities and in
fields, in palaces and in sewers,
in the holds of merchant vessels,
in the thatch of peasant roofs.
Where human beings build, store, and
trade, the black rat follows, and has done
so for thousands of years, and continues
to do so into the twenty-first century.
it was the ideal host for the black rat
flea, Xenopsylla cheopis, the species
that would serve as Yersinia pestis'
primary vector into human populations.
As infected rodent populations along
the trade routes began to collapse under
the weight of the epizootic plague,
it was the black rat already living in
waypoints, already nesting in the grain
sacks loaded onto merchant camels, already
stowed away in the bales of silk and
spice, moving steadily westward along the
Silk Road, carrying the fleas with it.
. Unknowingly, inevitably, mile by
mile, city by city, port by port,
the plague marched on relentlessly.
The most devastating pandemic in human
history did not ride into Europe on
horseback or sail under a dark flag.
It arrived in the hold of a merchant ship,
in the fur of a rat that no one noticed,
carried by a flea too small to see, and it
had been traveling for a very long time.
Before we follow the plague westward
to the Black Sea and the gates of
Europe, we need to pause because there
is an habit, deeply embedded in the
way history has traditionally been
written and taught, at least in the
Western world, of treating the Black
Death as a fundamentally European story.
The plague arrives in Sicily, the plague
kills a third of England, the plague
empties the cities of France, and the
rest of the world exists in this telling
as a kind of offstage backdrop, the
mysterious east from which the catastrophe
emerged before the real story begins.
While the Black Death did have a profound
impact on Europe, and documentation of the
plague is much more voluminous in European
records There is a bit of historical bias
that needs to be acknowledged because the
plague struck the rest of the world also.
The truth is that before Yersinia pestis
ever reached the Mediterranean, it had
already been moving through the most
populous region on Earth for decades,
and what it did there, what it took, what
it destroyed, what it erased deserves to
be spoken aloud, even if the historical
record does not always make that easy.
The numbers, imprecise
are still staggering.
In the early thirteenth century,
China's population stood at somewhere
above one hundred and twenty million
people, a civilization of extraordinary
sophistication and density, the
largest, and in many respects,
the most advanced on the planet.
By the end of the fourteenth century,
that population had fallen to somewhere
in the region of seventy million people.
Fifty million people gone.
Fifty million human lives extinguished
over the span of roughly two hundred
years, an entire civilization
diminished by more than a third.
The plague was not the only
author of this catastrophe.
The Mongol conquests of the thirteenth
century were themselves devastating,
campaigns of extraordinary violence
that depopulated entire regions
through slaughter, forced displacement,
and the deliberate destruction
of agricultural infrastructure.
Famine, driven by the same climatic
disruptions that were starving Europe,
compounded the suffering further.
War, flood, drought, and social
collapse were all pressing down
simultaneously on populations
already stretched to their limits.
But the plague was there, moving
through it all, and scholars who have
studied the period with increasing
rigor in recent decades are increasingly
persuaded that Yersinia pestis was
a significant and perhaps decisive
contributor to the demographic collapse.
Not the only blade, but
one of the sharpest.
The documentary record in China and
Central Asia is thinner than what
survives from Europe, a fact that
itself reflects the scale of the
destruction, since its scholars and
the scribes and the administrators
who kept records, they were dying,
too But what does survive is telling.
Chinese chronicles from the
mid-fourteenth century describe
outbreaks of mass death with a speed
and character entirely consistent
with pneumonic and septicemic plague.
Regional histories record
localities losing the majority of
their populations within weeks.
Administrative records simply
cease, not because the situation
improved, but because there
was no one left to write them.
There is also evidence
in the landscape itself.
Archaeologists and historical
geographers studying this period have
identified patterns of agricultural
abandonment, urban contraction, and
trade route disruption across Central
Asia and China that mirror with
striking consistency what we see in
plague-affected regions of Europe.
These are not the fingerprints
of war or famine alone.
They are the fingerprints of something
biological, something that moved
along routes of human contact and
killed at a speed and scale that
outpaced many a military campaign.
They suffered and died with no
framework whatsoever for understanding
what was happening to them.
In landscapes so vast and so
sparsely documented that their
individual suffering has been
almost entirely swallowed by time.
They were, as much as any English peasant
or Florentine merchant, human beings
Trapped in a biological vice beyond
their understanding or control, they
loved their children and feared death
and prayed to whatever they believed
in for relief that did not come.
We do not know their names.
We do not have their chronicles.
We cannot visit their memorials
because most were never built.
But they were the first, and they
deserve to be remembered as part of
the story, not as a prologue, not as
a geographical context, but as people,
as casualties, as human beings to whom
something terrible happened in the
shadow of the mountains, on the banks
of rivers that most of the world will
never see, at the beginning of the
worst pandemic humanity has ever known.
Let us move on to explore the actual
launching pad of the plague's movements
into the Western world In 1266, the
Republic of Genoa negotiated trading
rights on the Crimean coast from
the Mongol Khan of the Golden Horde
and set about transforming what had
been a modest fishing village into
something altogether more ambitious.
Over the next eight decades, Kaffa
grew with remarkable speed, and by the
1340s, it was a city of an estimated
75,000 inhabitants, enclosed within
fortified walls, bristling with
warehouses and counting houses, and the
infrastructure of international trade.
Its geographical position was,
for the purposes of medieval
commerce, close to perfect.
Sitting at the northern edge of the
Black Sea, it commanded the natural
gateway between the overland trade routes
of the Mongol Empire, the Silk Road
and its tributaries, and the maritime
routes of the Mediterranean world.
Everything that moved between East and
West, between the steppes of Central Asia
and the markets of Europe and Palestine,
had reason to pass through Kaffa.
Silk from China, spices from the Indies,
timber from the forests of the Black Sea
hinterland, furs from the Russian north,
worth fortunes in fashion-conscious courts
of Western Europe, and slaves, thousands
upon thousands of enslaved people,
captured on the steppe in the Caucasus
and sold on the docks of Kaffa to buyers
from across the Mediterranean world.
A trade in human beings so
extensive and so normalized that
it barely merited comment in the
commercial records of the time.
Heavy-laden Genoese galleys left
Kaffa's harbor in a constant stream,
fanning out across the Black Sea
and into the Mediterranean beyond.
Some sailed south to Constantinople,
the great Byzantine capital, which is
at the crossroads of two continents
in a city of a half a million souls.
Others made for ports of Anatolia, or
turned further south toward Alexandria
and the markets of Egypt and Palestine.
But most, the majority set their prows
westward through the Bosporus and the
Aegean, through the narrow Strait of
Messina, and into the heart of the
Mediterranean, to Italy, to France,
to Spain, to the wealthiest, most
densely populated, most commercially
connected cities in the Western world.
Caffa was not merely a trading post.
It was a pump drawing the products of
the East into its harbor and pushing
them westward into Europe with the
regularity and power of a beating heart.
And by the mid-thirteen fortys, Yersinia
pestis had arrived at its doorstep.
The precise pathway by which the
plague reached Caffa is one of the most
dramatic episodes in medieval history,
and one that deserves its own moment
because it introduces a dimension
to this story that no account of the
Black Death can responsibly ignore.
In thirteen forty-six, the Mongol army
of the Golden Horde under the command
of Khan Jani Beg laid siege to Caffa.
The reasons were commercial and political.
A dispute over trading rights and the
killing of Mongol merchants had inflamed
relations between the Genoese city and
its Mongol overlords, and the Mongols had
resolved to settle the matter by force.
The Genoese, protected by their fortified
walls, settled in to wait the siege out.
But the Mongol army besieging
the city was already dying.
Plague had reached the Golden Horde's
forces, moving along the same steppe
routes and trade networks we have already
described, and was cutting through the
besieging army with devastating speed.
Men who had been healthy at sunrise
were often dead by nightfall.
The camp outside Caffa's walls was
becoming rapidly an open graveyard.
And then Jani Beg or his
commanders made a decision that
would echo through centuries.
They began catapulting the plague-ridden
corpses of their dead over the
walls of Caffa and into the city.
The Genoese defenders, already
under pressure, now found themselves
besieged not only by the Mongol army
outside their walls, but by something
far more dangerous within them.
The plague took hold inside the city.
The Genoese did what people have always
done when a place becomes uninhabitable.
They fled.
Merchants, traders, sailors, men with
ships and the means to use them abandoned
Kaffa and took to their galleys, desperate
to put distance between themselves and the
horror unfolding in the city behind them.
They sailed south and west, following
the routes they knew, calling at the
ports along the way, Constantinople,
Messina, Genoa, Marseille, Valencia.
They did not know as they pulled away
from the docks of Kaffa that they were
carrying the plague with them, that
the men hauling the rigging and working
the oars were already some of them
incubating a disease that would kill them
before they reached their destination.
They knew they only had to
leave, and so they left.
In October of 1347, a fleet of Genoese
trading vessels sailed into the harbor of
Messina on the northeastern tip of Sicily.
When the harbor masters and
the dockworkers of Messina
approached the ships, they
were shocked to see dead sailors,
and those still alive were in a
state of advanced and terrible
illness, delirious with fevers,
and the reek of death on the ships.
The authorities of Messina ordered the
fleet out of the harbor immediately.
But as you've gathered
already, it was too late.
The rats had disembarked.
The fleas had found new hosts.
The bacterium was already moving through
the port town, through the markets
and the alleyways, and the tightly
packed housing of a busy medieval city.
Within days, people in Messina
were dying with the same terrible
symptoms as the sailors on the ships.
Within weeks, the plague had
spread to Catania, to Palermo,
to the Sicilian countryside.
Within months, it was
on the Italian mainland.
The Black Death had arrived in Europe,
not with a declaration, not with a
warning, not with anything that could
have been seen or stopped or understood.
It arrived the way the most devastating
things always arrive: quietly,
invisibly, carried by ordinary people
going about their ordinary lives
We've looked at about where the plague
came from, how it traveled, how it
arrived, but we need to talk about
what it actually did, what happened
inside the body of a plague victim
Because to understand why the
Black Death killed so many people
so quickly, you need to understand
something that is both scientifically
fascinating and deeply unsettling.
Yersinia pestis is not simply
a bacterium that makes you ill.
It is a bacterium that has
evolved a suite of biological
weapons specifically designed to
defeat the human immune system.
The moment of infection is
almost insultingly small.
A flea, Xenopsylla cheopis, the black
rat flea, no larger than a sesame seed,
lands on exposed skin and begins to feed.
Recall what was said earlier.
Yersinia pestis blocks the
flea's digestive tract, causing
a kind of biological desperation.
The flea feeds ravenously and repeatedly,
unable to satisfy its hunger, and
with each bite, it regurgitates the
bacteria-laden material directly into the
wound, and the infected person feels at
most a small itch, a minor irritation,
nothing that would cause alarm.
And yet, in that moment,
the invasion has begun.
The bacteria enters the lymphatic
system, the body's own internal drainage
network, a highway of vessels and
nodes that runs throughout the body and
plays a central role in immune defense.
Under normal circumstances, the
lymphatic system is exactly where
you want a pathogen to end up.
It is, in effect, the body's first
line of organized resistance, the
place where immune cells gather to
identify and destroy foreign invaders.
But Yersinia pestis has been playing
this game for a very long time,
and it has evolved and learned.
The bacteria travel along lymphatic
vessels towards the nearest lymph
node, typically in the groin if
the flea bite was on the leg, in
the armpit if on the arm, in the
neck if on the face or upper body.
And here, rather than being destroyed,
they suppress the immune response,
something pretty remarkable.
Yersinia pestis carries a toolkit
of molecular mechanisms, proteins,
and enzymes that actively disable
the immune cells sent to kill it.
It prevents the signals that would
normally summon reinforcements.
It hides inside the very cells designed
to destroy it, replicating quietly while
the body's defenses are still trying to
understand what they are dealing with.
This is why the first day or
two of the plague infection
can feel deceptively mild.
A headache, a fever, fatigue.
The body knows something is
wrong, but its response is being
actively suppressed and undermined.
The enemy is already inside the walls,
and alarm bells are being muffled.
Then the lymph node begins to fail.
Overwhelmed by the replicating bacteria,
flooded with the debris of a losing
immune battle, the lymph node swells
rapidly, dramatically, painfully.
This is the bubo we talked about.
The word comes from the Greek groin,
as mentioned before, though bubos can
appear wherever the lymph nodes are
largest and closest to the skin's surface.
They grow with alarming speed, up to
the size of an egg, sometimes larger,
within twenty-four to seventy-two hours.
The skin over them stretches and
discolors, moving from red to a deep
angry purple as the tissue underneath is
damaged and the blood vessels around the
lymph node hemorrhage under the pressure.
The pain is, by all historical
accounts, extraordinary Medieval
physicians who had the courage to
examine plague victims described the
buboes with a mixture of clinical
fascination and barely concealed horror.
Some attempted to lance them.
A few patients, it was recorded, survived
this intervention, but most did not.
But the bubo, is not what
kills most plague victims.
What kills them is what comes next.
If the immune system cannot contain
Yersinia pestis within a lymph
node, and in the vast majority of
untreated cases, it cannot, the
bacteria breaches the node's walls
entirely and enters the bloodstream.
This transition marks the outset
of septicemic plague, and it
represents a catastrophic escalation
in the severity of the infection.
Once in the blood, Yersinia pestis
triggers a response that is, in a
terrible sense, self-destructive.
The immune system, finally recognizing
the scale of the invasion, responds
with everything it has, releasing
a massive cascade of inflammatory
signals, a biological alarm so loud
and so overwhelming that it begins
to damage the body's own tissues.
The immune system, in its desperation,
essentially losing control and
attacking indiscriminately.
Blood vessels throughout
the body begin to leak.
Tiny hemorrhages erupt beneath the
skin, the dark spreading blotches
that gave the Black Death one of its
names, visible as bruise-like patches
across the torso, the limbs, the face.
The body's clotting mechanisms go
into overdrive, forming microscopic
clots in the small blood vessels
throughout the organs, simultaneously
clotting where it shouldn't and
failing to clot where it must.
This is commonly known as DIC,
disseminated intravascular coagulation.
Blood pressure falls, organs begin to
fail, starved of the oxygen they need,
the kidneys, the liver, the brain.
The extremities, fingers, toes,
the tip of the nose are cut off
from their blood supply entirely
and begin to die, to turn black.
This is tissue necrosis,
gangrene developing in real
time on a living person.
It is among all the symptoms of plague,
perhaps the most visually striking
and most symbolically resonant.
The body, still alive,
already beginning to rot.
And through all of this, the fever, the
agony of the buboes, the hemorrhaging,
the organ failure, the bacteria continue
to replicate doubling again, flooding
the bloodstream in the numbers the
immune system cannot begin to match.
For most untreated patients reaching
this stage, death comes within
days, sometimes within hours.
But Yersinia pestis had one more
form to reveal, one more way to kill.
And this one, the third and most lethal
manifestation of plague, was the one
that made containment Not just difficult,
but often impossible in a medieval world
with no understanding of contagion.
When the bacteria reaches the
lungs, either by spreading through
the blood or inhaled directly from
the cough of another victim, the
infection becomes pneumonic plague.
And pneumonic plague changes
the entire nature of the threat.
In the lungs, Yersinia pestis triggers a
severe and rapidly progressing pneumonia.
The air sacs fill with fluid, with
bacteria, with the wreckage of
immune cells fighting and losing.
Breathing becomes labored.
A wet, increasingly desperate
struggle as the lungs lose their
capacity to exchange oxygen.
The cough that develops is productive
and bloody, pink at first, then darker
as the damage to lung tissue deepens.
And with every cough, the infected person
expels a fine aerosol of bacteria-laden
droplets into the air around them.
This is the moment at which the plague
stops being a disease of flea bites
and becomes a disease of breath.
No flea is required, no rat,
no physical contact at all.
Simply breathing the same air as a
pneumonic plague victim, standing close
enough to hear them speak, leaning over
them to offer water, sitting beside them
to pray, is sufficient for transmission.
The bacteria travel on the breath
from one pair of lungs to another,
bypassing the flea entirely,
bypassing every barrier that might
otherwise have slowed the spread.
Pneumonic plague, untreated, kills
virtually everyone it infects.
100% mortality.
Kills fast, often within 24 to 48 hours
of the first respiratory symptoms.
Victims who were walking and
talking in the morning could
be dead by the following dawn.
For the people of medieval Europe, with
no germ theory, no antibiotics, no concept
of airborne transmission, no protective
equipment of any kind, this was an enemy
they could not see, could not understand,
and had almost no means of fighting.
Their most instinctive response, to
gather around the sick, to tend to them,
to offer comfort and human presence
in the final hours, was tragically and
cruelly precisely the behavior most
likely to spread the disease further.
The plague did not merely
exploit human biology.
You could say it exploited human
love, and that perhaps more than
any other single fact explains
how it killed so many so fast.
We began this episode with a question:
Why should we care about the Black Death?
We have, I hope, begun to answer it.
We have followed Yersinia pestis from
the burrows of the Mongolian steppe,
along the Silk Road, through the dying
camps of the Mongol army, outside the
walls of Kaffa, and into the holds of
Genoese trading ships heading west.
We have watched at the most intimate
biological level what Yersinia pestis
actually does, how it deceives, how it
disables, how it turns the body's own
defenses against itself, and how it found,
in the warmth of human compassion, in the
instinct to sit with the sick, to hold the
dying, to stay when every rational impulse
screamed to run, its most effective and
most heartbreaking route of transmission.
We know of some 50 million souls lost
in China and Central Asia before the
plague ever reached European shores.
We have sat briefly with an English woman
in the final days of her life and tried
to give some small measure of reality
to what those days would have felt like.
The fever, the swellings, the
hemorrhage, the slow and terrible
drowning in her own lungs.
These things happened to real
people who were, in every way
that matters, exactly like us.
And this story is not finished.
In part two of this podcast, we follow
the Black Death as it moves northward and
westward through the body of Europe, from
the sun-baked ports of Sicily and the
great trading cities of Italy, through
the mountain passes of the Alps, across
the vineyards and cathedrals of France,
over the channel to England and beyond.
We will look at how medieval European
society responded to this catastrophe
unfolding around it, from the learned
physicians who tried and failed to
make sense of what they were seeing,
to the flagellants who walked bleeding
through the streets of German cities,
whipping themselves raw in an act of
collective penance, to the mobs who
turned on Jewish communities across Europe
with murderous and entirely baseless
accusations of deliberate poisoning.
We will also look at what was built
in the ruins, because the Black Death,
for all its devastation, was one of the
great catalysts of the modern world.
The feudal system, already
creaking, did not survive it intact.
The authority of the church, shaken
to its foundation by a catastrophe
its theology could not adequately
explain, would never fully recover.
The survivors, the laborers, the
peasants, the people at the very
bottom of medieval society found,
for the first time in generations,
that their labor had become scarce.
And scarcity, as it always
does, created power.
